Community Aged Care Packages
The AVWA provides Community Aged Care Packages (CACPs) to frail older Vietnamese people in the Western and Northern Regions of Melbourne, currently there are 50 packages for the West and 25 packages for the North including brokerage services to other service providers.
 
A Community Care Package is a planned and coordinated package of community care services to assist a person who requires management of services because of their complex needs.
 
These CACPs are targeted at frail older Vietnamese people living in the community. These people would otherwise be eligible for at least low residential care.
 
The aim of the CACPs program is to provide flexible and coordinated support through a case management approach to frail older Vietnamese who wish to continue living in their own homes and participating in the life of their community.
 
The program acknowledges that older people from culturally and linguistically diverse backgrounds have rights for services that appropriately respond to their cultural and language needs.
 
All our staff are well qualified, most of them are bilingual and some are trilingual and all care workers hold at least Certificate III in Aged Care.
 
The CACPs service provides personal assistance for any of the following:
 

    *      bathing, showering, or personal hygiene
    *      toileting
    *      dressing or undressing
    *      mobility
    *      preparing and eating meals
    *      laundry
    *      home help
    *     emotional support and direct supervision
    *      rehabilitative support
    *      transport to help the client shop, or to attend doctors or hospital appointments
    *      temporary respite care in the home
    *      advocacy services
    *      providing transport to social activities
