Become Our Care Worker Coordinator

We are seeking a dedicated and qualified Care Worker Coordinator to join our team. You will be responsible for supporting and assisting Home Care Packages (HCP) program in meeting all clients' diverse and complex needs whilst maintaining high level compliance and customer satisfaction in delivering care and services. Your responsibilities also include guiding, supporting, and supervising Care Workers, as well as liaising with internal teams and external service providers for quality service delivery. 

Location:1 position based in Springvale/ Richmond, 1 position based in Braybrook

Reports to: HCP Regional Manager Department/Program: Aged and Disability Services

What we are looking for:

  • Coach and supervise Care workers and Lead Care workers:
  • Lead the team to ensure compliance with Aged Care Quality Standards and AVWA policies and procedures.
  • Provide leadership and support to the Care workers to ensure high quality and safe direct care that meets clients' needs.
  • Maintain an atmosphere of courtesy and respect and an environment of support and kindness.
  • Work together with HR team:
    • Care worker recruitment process: review applications, conduct interview, induction  
    • Take action after receiving report from HR regarding Care worker’s health and well-being (including attending welfare meeting, ….)
    • Conduct 6-month probation performance appraisal for new Care workers
    • Arrange & conduct annual performance appraisals for Care workers and Lead Care workers
  • Build the development plan for Lead Care workers and Care Workers, and ensure that performance is regularly monitored.
  • Coach, support, and supervise Lead Care workers - Support, supervise Care Workers - Monitor the performance of Lead Care workers and Care Workers
  • Handle feedback from Care workers - Work with Rostering team to manage Care workers’ leaves effectively Coordinate team support and foster a positive working environment.
  • Respond to emergencies and assist with COVID-19 reports.
  • Collect and review statistical data and program records.
  • Collaborate with the Rostering team and handle client feedback/complaints.
  • Plan and facilitate team meetings and propose training opportunities.

Who You Are:

  • Tertiary qualifications in health-related fields or relevant experience.
  • Current First Aid/CPR certificates.
  • Knowledge and experience in Aged Care services, including Home Care Packages and Consumer-Directed Care approach.
  • Management and leadership experience. - Sound understanding of aging process, aged care system, interventions, and support services.
  • Strong prioritization, multitasking, and issue resolution skills.
  • Proactive, results-driven approach with excellent communication skills.
  • Ability to maintain professional community networks and promote AVWA's programs effectively.
  • Commitment to fostering a culture of best practice service delivery and effective team communication.
