Reports to:  HCP West Coordinator

Role purpose

We are looking for a full-time HCP Care Manager who will collaborate with the customers and their family to provide high-quality care management for customers receiving AVWA Home Care Packages. To provide supervision and support to Care Workers. To liaise and work with other health care providers to ensure coordinated care on a Consumer Directed Care basis, including referring consumers for clinical assessments and/or assessments of cognitive impairment and dementia, and assisting them with goal achievements.

What we are looking for:

  • Responsible for the provision of care management to consumers on the Consumer Directed Care basis,
  • Acts in a professional manner at all time, ensuring that optimum, flexible individualised care is responsively delivered to all consumers, through a process of consultation, assessment, and care planning,
  • Conducting an initial assessment for HCP consumers and home safety assessment
  • Developing care plans and individualised budgets for consumers in partnership with them and/or their representative
  • Evaluate individual goals and service outcomes
  • Conducting formal re-assessments, including referring to other health care providers of clinical assessments of consumer needs and adjusting care plans and individualised budgets and required
  • Maintaining up-to-date consumers records, agreements, and care plan
  • Monitoring equipment supplies and ensures items are used efficiently and effectively.
  • Reporting all incidents to the Coordinator and implementing appropriate action
  • Providing supervision and support to Care Workers
  • Working with Rostering team to monitor and update Care Workers’ roster, prepare duty statements and brief Care Workers about the required tasks

Who you are:

  • Appropriate tertiary qualifications in Social Work, Care Management, health disciplines, human services field or equivalent.
  • Valid First Aid and CPR certificate.
  • Extensive experience working with older people and/or with Home Care Packages consumers
  • Demonstrated experience and expertise in working with Vietnamese aged people and their families, including counselling, assessment, and care management
  • Demonstrated knowledge of the aging process and aged care system, eg. My Aged Care, community health care, allied health services
  • Demonstrated knowledge of available interventions, support services, and the appropriate referral networks
  • Excellent communicator and ability maintain strong stakeholder engagement
  • Fluent in Vietnamese and English
